Blue Skies Pet Care, LLC – Dog Training Service Agreement

I, the Client, certify that my pet(s) are in good health and have not exhibited aggressive behavior towards people or other pets unless disclosed for dog training services. I agree to the following:

I. The term Blue Skies Pet Care, LLC as used herein refers to the officers, owners, principals, employees, and subcontractors of the company known as Blue Skies Pet Care, LLC, and its authorized representatives.

II. Regarding dog training, sessions canceled or rescheduled by the Client with fewer than 48 hours notice will count towards the total number of sessions. All sessions must be completed within two weeks of the original end date of the training package or they will be forfeited. No refunds will be offered if the client chooses to discontinue the program.

III. Payment Policy: Client agrees to review scheduling and invoices via the Scheduling and Invoices webpages of their online portal (https://www.timetopet.com/portal) and report any necessary changes prior to services being rendered via portal messaging or by calling the Blue Skies Pet Care, LLC office. Client agrees to pay Blue Skies Pet Care, LLC the total fee for dog training services prior to or on the first day of service unless otherwise agreed to in writing. Any sessions canceled or rescheduled by the Client with fewer than 48 hours notice will still be charged towards the total fee.

IV. Accounts not paid by invoice due dates are subject to an "After Due Date Balance" of the original invoice balance plus 10%. Open invoices will be resent monthly with a 2% monthly delayed payment fee.

V. Payment can be made in cash, check made payable to Blue Skies Pet Care, LLC, or through the Blue Skies Pet Care, LLC online portal. Client agrees to pay any additional charges due upon completion of the service.

VI. Blue Skies Pet Care, LLC will endeavor to create as safe an environment as possible for the training and care of pets and will offer only sound, safe, and responsible training, and post-training instructions. However, Client recognizes that Blue Skies Pet Care, LLC is not responsible for any unintentional errors, omissions, or incorrect assertions. Client understands that the recommendation of any other product or service is not a guarantee of satisfaction with that product or service. Further, Client is and will remain responsible for the actions of their pet at all times and Client hereby agrees to indemnify and hold harmless Blue Skies Pet Care, LLC of any and all claims of injury, expense, costs, or damages caused by the actions of the Client's pet while under Blue Skies Pet Care, LLC care and under Client's own care as a result of following training instructions. Client understands the inherent risks in owning a dog, including but not limited to the risk of dog bites to the Client or others. Blue Skies Pet Care, LLC agrees to provide the services stated herein in a reliable and trustworthy manner. In consideration of these services, and as an express condition thereof, the Client expressly waives and relinquishes any and all claims against Blue Skies Pet Care, LLC.

VII. The inaugural term of this contract shall be from initial date of service until revoked in writing. Blue Skies Pet Care, LLC reserves the right to terminate this contract at any time without prior notice. Any changes or amendments to this contract can be made at any time without prior notice. Rates and services are subject to change without prior notice.

VIII. Client hereby grants Blue Skies Pet Care, LLC permission to use their property and pet’s likeness in a photograph or video in any and all of its publications, including website entries, without payment or any other consideration. Client understands and agrees that these materials will become property of Blue Skies Pet Care, LLC.

IX. Client acknowledges that any vehicle driven by a representative of Blue Skies Pet Care, LLC may be equipped with advertisements including the business name and agrees to allow this vehicle to park on or near the site of services (i.e. home, office, etc).

X. This contract authorizes Blue Skies Pet Care, LLC to enter the Client’s home and property to provide the services listed herein.

XI. Key Policy: Blue Skies Pet Care, LLC prefers to have two modes of access to client premises, such as a garage code and a lockbox, two keys, or similar arrangements. This ensures reliable access for scheduled sessions even if one mode fails or is unavailable. It's important to note that if Blue Skies Pet Care, LLC is scheduled to conduct a session when the client is not present, and access to the premises fails or is unavailable, resulting in our inability to reach the client for timely access, it may still count as a completed session. Blue Skies Pet Care, LLC will discuss and agree upon the preferred modes of access during the introductory meeting with the client.

XII. If the pet(s) become ill while under the care of Blue Skies Pet Care, LLC, and medical care is needed in the best judgment of Blue Skies Pet Care, LLC, Client authorizes Blue Skies Pet Care, LLC to transport the pet(s) to Client’s veterinarian (or one who is available). Client authorizes Blue Skies Pet Care, LLC to approve any emergency treatment recommended by said veterinarian. Client authorizes Blue Skies Pet Care, LLC to take the Client’s pet(s) to an Emergency Clinic for Animals when the Clients veterinarian is not immediately available. Client further agrees to promptly reimburse Blue Skies Pet Care, LLC for any expenses incurred for emergency treatment. Client releases Blue Skies Pet Care, LLC from any and all liabilities related to transportation, treatment, and expense resulting from emergencies. Client recognizes that Blue Skies Pet Care, LLC cannot be held responsible for injury or death of pet(s) related to veterinary treatment.

XIII. Client attests that all pets that will be involved in dog training or other services have rabies vaccination that are not exempt due to health reasons are up to date and agrees to provide Blue Skies Pet Care, LLC with a copy of or access to current veterinary records stating the date of the most recent vaccinations by request.
